This 3-week online course reviews current evidence and practical approaches to three common neurological emergencies in veterinary patients. We will cover traumatic brain injury, status epilepticus, and spinal trauma and acute disc disease. Each week focuses on triage, diagnostic decision-making, medical and surgical options, and monitoring. The content is designed for veterinarians who want the latest updates, with clinically applicable protocols you can use in practice.

Course Outline

We open with traumatic brain injury, focusing on acute patient stabilisation. The primary goal here is the prevention of secondary brain injury through targeted interventions, including ensuring optimal oxygenation, cerebral perfusion, and intracranial pressure control. We will review the evidence for hyperosmotic agents, management of spreading micro-depolarisations, and post-trauma seizure prophylaxis. Case examples will illustrate how to stage severity and when to escalate or wean care. You will come away from this week with a sound understanding of the approach, monitoring and interventions required to help achieve the best outcome in your traumatic brain injury patients!

This week covers definition, classification, and stepwise treatment of status epilepticus. We will evaluate the literature and optimum care using first-line anti-epileptics, when to start second-line IV anticonvulsants, adjunctive therapies, and anaesthetic and ventilation options for refractory cases. The discussion includes diagnostic workup, metabolic considerations, and when to pursue advanced imaging. We will also address owner communication regarding prognosis and long-term management. A strong emphasis will be provided on time-sensitive decision-making, drug selection and monitoring, as well as achieving patient stability – designed to make these challenging cases less stressful.

The final week addresses acute spinal cord injury, focusing on traumatic fractures/luxations and Hansen Type I disc extrusion. We will review the neurological localisation and grading systems that guide urgency and treatment choice. Medical management, when to refer for surgical decompression, imaging interpretation and timing of critical decisions are key steps to be covered. We finish with patient physical care fundamentals and prognosis based on initial presentation and 48-hour response.

Learning Outcomes

By the end of this course, participants will be able to:
  1. Apply the latest literature and evidence to the management and stabilisation of the patient with traumatic brain injury.
  2. Apply the latest evidence-base to select appropriate antiepileptic drug and supportive care protocols for the management of status epilepticus – as well as management of the post-ictal patient.
  3. Differentiate between acute non-surgical and surgical spinal conditions – including traumatic spinal injury – based on clinical signs and imaging findings.
  4. Develop monitoring plans that detect early deterioration in each of the conditions discussed in the course.
  5. Adjust treatment based on findings of serial neurological examinations and objective parameters, using the latest evidence-base.

Dr Philip Judge

BVSc MVS PG Cert Vet Clin Stud MACVSc (Vet. Emergency and Critical Care; Medicine of Dogs)

Philip graduated from Massey University in New Zealand in 1992, and spent 7 years in small animal practice before undertaking a 3-year residency in veterinary emergency and critical care at the University of Melbourne in 1998.

Following his residency, Philip worked for nearly 6 years at the Animal Emergency Centre in Melbourne, becoming the Senior Veterinarian at the centre in 2004. In 2006, Philip undertook a 1-year surgical externship before moving to Townsville to take up the position of Senior Lecturer in Veterinary Emergency and Critical Care at JCU. Philip is also co-founder, and director of Vet Education Pty Ltd (www.veteducation.com) – one of Australia’s leading providers of online continuing education for veterinarians and veterinary nurses.

Philip has published numerous manuals and guides concerning emergency medicine, including a CRI manual, haematology and biochemistry interpretation guide, emergency anaesthesia guide, and a ventilation therapy manual for small animals, in addition to being published in peer reviewed literature.

Philip’s key interests in veterinary science include respiratory emergencies, ventilation therapy, envenomations and toxicology.
